We have an exciting opportunity to take on the role of Senior Support Worker within our Floating Support Service based in North Camden, which provides support to adults with mild-moderate learning disabilities and a range of other needs, including mental health and complex health needs.

The role involves assisting with management of the day-to-day running of the service, coordinating person-centred care for service users and overseeing the development and implementation of care plans designed to provide meaningful choice and increase independence and participation.

Working in partnership with service-users and their families, you will actively participate in the aim of the service, which is to enable service users to live independently in their own home, to engage in meaningfully daily activities, to achieve their desired outcomes and to reduce reliance on 1:1 support.
