The support we provide is tailored to the individual’s needs and preferences and, as with all Centre 404 services, we aim to support our service users to develop independent living skills including:
We focus on providing an active model of support derived from Positive Behaviour Support principles, which enables individuals to make positive life changes, achieve goals, increase independence, and improve self-esteem.
The focus in outreach services has always been to help develop strong local connections with various groups and networks, ensuring that people we support remain safe whilst maintaining their physical and mental well-being. Our activity offers for our service users are a blend of both in person and virtual activities.
Our experienced and dedicated staff team, many of whom have been with the service for a number of years, visit service users in their own homes or meet them out in the community at times that suit our service users.
Our staff are trained in using strength-based support by keeping the person they support at the heart of decision making. Staff use the principles of ‘Making Every Contact Count’ and ‘3 Conversation Model’ to support each individual holistically, and target support to the needs of the person rather than offering generic support. This in turn helps each individual feel more empowered and enables them to become full and equal members of society who have their voices heard.
Centre 404 and Islington Learning Disability Partnership (ILDP) worked together to secure a date for his eye surgery, which was delayed by almost two months. Despite this, Peter remained positive, and the surgery in early October was a success, restoring his vision. With his eyesight better than ever, Peter quickly regained his independence.
Following her sister’s recommendation, Rebekah’s mother consulted Centre 404’s Manager, who suggested a team of key workers tailored to Rebekah’s specific needs. Since January, Centre 404’s staff have been supporting Rebekah, bringing new energy and strengths to her daily life.
He enjoyed the friendly and welcoming atmosphere, prompting him to express his interest in applying for a job there. Onur and his dedicated support worker, Fatima, requested an application form, but filling it out proved to be a challenge. Despite his frustration, Onur’s determination to apply for the job continued.
