Project Manager (Outreach & Supported Living) 

 

Salary:  £28,500 per annum

Hours: Full-time, 37 hours per week

Contract: Permanent

Centre 404 is currently looking for a driven and efficient individual to manage a supported living project for 9 adults, as well as a growing outreach service for more than 40 service users.  Both services provide support to adults with mild-moderate learning disabilities and a range of other needs, including mental health, Autism and complex health needs.

 

The role will include coordinating and providing excellent person-centred care and support to service users, enabling them to live independently in their own home and be supported in all areas of day-to-day living. You will oversee the development and implementation of care plans designed to provide meaningful choice and a range of activities and support to meet people’s individual needs. As Project Manager, you will also contribute to the development of the team and be responsible for encouraging a culture based on respect and dignity for those in need of care and support, providing supervision and clear management to a team of Support Workers.

 

The ideal candidate will have experience of leading and coaching a team supporting people with learning disabilities or another vulnerable client group. Experience of supporting people with high and complex support needs would also be an advantage.
